COURSE DESCRIPTION
How do therapists move beyond being "affirming," to provide USEFUL care to clients seeking a letter for gender-affirming medical care?  How do we navigate the historical traumas of our profession, the mandated attendance in our sessions, and build a trusting relationship with a client? How do we understand our role when a client does not want therapy, just a letter? How are we helpful and knowledgeable about the next steps, when clients are often more researched than we are?

This course will share valuable insights from over 5 years of intensive work with transgender and non-binary clients seeking letters for gender-affirming medical procedures.  In this course, participants will learn how to facilitate a standalone assessment for gender-affirming surgery, hormone therapy, and pubertal suppression using informed consent and healthcare trauma-informed frameworks.  Participants will be exposed to the historical and current context of our role in medical care and build practices that can minimize harm.  Participants will learn to address factors that can complicate, or be complicated by, medical care; review the risks and expectations for each medical procedure; and support clients through the steps of medical preparation and aftercare.  Participants will be exposed to the fundamental dynamics of each gender-affirming surgery and hormone intervention that is currently covered by insurance, and learn about the provider options and wait-times in the Portland metro area.

LEARNING OBJECTIVES
Define WPATH, insurance, and provider requirements for quality letters and assessments.
Establish trust with clients through an accountable and trauma-informed  consent dialogue.
Conduct a mental health assessment for gender affirming surgery focusing on experiences relevant to medical care.
Present the expectations for chest reconstruction, breast augmentation, metoidioplasty, orchiectomy, hysterectomy/oophorectomy, phalloplasty, electrolysis, and vaginoplasty; as well as testosterone, estridol, progesterone, spironolactone, bicaludimide, and pubertal suppression.
Have concrete tools for preparing clients for surgery.
Write a comprehensive letter of support that is consistent with WPATH standards of care, insurance and surgeons criteria, and is respectful to client privacy.


TRAINING MODALITIES
During this training, participants will engage in didactic presentation, self-reflection, case reviews, videos of clients experiences, and paired role-play. After the training, participants pursuing CE credit will submit 1-2 mock letters of recommendation for review and feedback.  Those who complete all parts of the training will be added to a vetted list of providers shared by Brave Space and community medical providers.

Participants will gain access to the Brave Space Surgery Planning Guide, a helpful tool for perioperative planning, quality letter and assessment templates, and the Powerpoint slide for future reference.

Participants are responsible for determining if CEs offered for this course meet board criteria for continuing education.  A CE certificate, facilitator bio, and course description will be provided to attendees to use for CE submission.
